New Student Registration
HOW DO I REGISTER MY CHILD IF I AM MOVING TO THE PITTSFIELD SCHOOL DISTRICT?
STEP #1: Make a visit or make an appointment to the school and bring with you the following required documents:
· Copy of immunization record showing all immunization dates
· Physical examination (Elementary School only – a physical examination can be accepted if it is not over one year old)
· Copy of birth certificate and any legal documentation of any subsequent name change
· Proof of residence – Copy of tax bill or rental contract is acceptable. If living with another family, a letter from the person with whom you are living along with his/her tax bill or rental contract.
· When parents are separated or divorced, a copy of custody documentation and/or parenting plan
Also helpful, but not required:
· Copy of transcript or most recent report card
· Copy of Individual Education Program (I.E.P.)
· Copy of Section 504 Accommodation Plan
· Copy of checkout form from previous school with current grades
STEP #2: Complete the following paperwork:
· Registration Form
· Home Language Survey
· Health Information Form
· Non Prescription Medicine Form
· Emergency Information
· Telecommunication Release and Authorization Form
· Permission to Release Records
· Disclosure of Directory Information Form
STEP #3: Participate in a meeting with:
· Elementary school: Principal
· Middle school: Guidance Counselor
· High school: Guidance Director
STEP #4: Complete the following paperwork:
· Family/Student Handbook Receipt
· Free and reduced lunch form (even if you think you are not eligible)
· Vehicle Registration Form (HS only)
· School/Family Compact (ES only)
· Kindergarten Information Forms (ES and K only)
STEP #5: If your child is student who has an Individual Education Program or Section 504
Accommodation Plan, a meeting will be scheduled within 30 days of your child starting school.
